Welcome to the Aktion Club Annual Achievement Report. This is where you can share about your club's achievement for the year. By completing the following questions, your club has the opportunity to earn either an "honor" or "distinguished" electronic badge in recognition of the year's accomplishments. Scoring information is indicated below.

Each "yes" answer earns one (1) point; "no" answers earn zero (0) points. This electronic report must be completed by October 15th to be considered for recognition. A link to access your club's electronic badge will be emailed by November 15 to the email address provided on this form. Thank you for participating in this award program!
